XXX. Pediatric

       A. ET tube depth is 6 + wt in kg.

       B. Many formulas exist for calculating the diameter of
          the endotracheal tube.  A formula that gives a close
          approximation is 2.5 x [wt in kg / 2].  Try this size
          first, but have both smaller and larger tubes in the
          room.  (i.e., for a 5 year old, have 4.5, 5, and 5.5
          uncuffed tubes)

       C. Children younger than 9 years old should have an
          uncuffed tube.

       D. Air embolism can occur in patients in supine position
          during surgery of the anterior skull. 

XXXI. Retromastoid Craniectomy

       A. No foley, limit fluids, A-line, no CVP,
          arrythmias ( mostly brady), ?hypertension,
          cranioplasty.
